ABSTRACT

OBJECTIVE@#To explore the correlation among gingival thickness (GT), underlying alveolar bone thickness (BT), and other periodontal biotype characteristics in the maxillary anterior.@*METHODS@#A total of 40 young volunteers with healthy periodontal were involved in this research. The periodontal probe was previously used to divide the gingiva from thick to thin. Two records were measured by cone beam CT (CBCT) GT, which was measured at the cement-enamel junction level; and BT, which was measured at 3 locations: 1, 3, 5 mm below the alveolar crest. Oral and gypsum measurements were used to analyze the associations of the crown width/crown length ratio (CW/CL), the keratinized mucosa width (KM), and the free gingival margin curvature.@*RESULTS@#Significant difference in the GT was observed between the thick and thin biotypes, which were divided by periodontal probe (P<0.01). Difference was observed in each periodontal biotype characteristic between the thick (GT≥1 mm) and thin biotypes (GT<1 mm) (P<0.05). BT was positively associated with GT (r=0.293, P=0.001), CW/CL (r=0.273, P=0.003), KM (r=0.291, P=0.001), and free gingival margin curvature (r=0.290, P=0.001).@*CONCLUSIONS@#The transparency of the probing in the sulcus could analyze the GT qualitatively. The thick and thin biotypes have different periodontal biotype characteristics. Compared with individuals with thick biotype, those with thin biotype are susceptible to risk dental aesthetic.

ABSTRACT

Objective To explore the predictive value of baseline CD4+ cell counts and CD4/CD8 ratio in immune reconstitution among HIV/AIDS patients receiving highly effective antiretroviral therapy (HAART). Methods A total of 90 HIV/AIDS patients who had received over 24 months of HAART regimen and had viral road s) (s) (200~500 cells/μl) and immunological responders (IRs) (>500 cells/μl) according to the CD4 cell counts after two-year HAART regimen. The characteristics and dynamic CD4+ cell counts among the three groups were compared, and the factors of immune reconstitution were analyzed. Results The CD4+ cell counts in the immunological responders (IRs) were significantly higher than those in the immunological non-responders (INRs) and immunological inadequate responders (IIRs) (P+ cells reached the highest predictive value at 1 year of HAART. Conclusion Our findings suggested that early initiation of HAART could reduce the failure of immune reconstitution. The combination of baseline CD4+ cell counts and baseline CD4/CD8 ratio may serve as a valid predictor of immune reconstitution prognosis after HAART.

ABSTRACT

Objective To evaluate the clinical value of total hemihepatic vascular exclusion (THHVE) for liver resection in hepatocellular carcinoma (HCC) patients and its effect on tumor recurrence, metastasis and patient survival. Methods Consecutive patients who were scheduled for elective hepatic resection were screened and allocated randomly to THHVE and Pringle maneuver groups. The total intraoperative blood loss, blood loss during transection, blood transfusion rate, operation time, vascular clamping time, complication, mortality, postoperative hospital stay, postoperative liver function index, overall survival time and diseasefree survival time were analyzed and compared between the two groups. Results From Aug. 2011 to Aug. 2013, 143 patients were eligible and were analyzed, with 71 in THHVE group and 72 in Pringle group. Baseline data were similar between the two groups. Total blood loss (250. 0 [150. 0-400. 0] mL vs 350. 0 [200. 0-637. 5] mL,P<0. 001) and blood loss during hepatic transection (100. 0 [50. 0-200. 0] mL vs 215. 0 [100. 0-380. 0] mL, P<0. 001) in the THHVE group were significantly less than those in the Pringle group. The vascular clamping time in THHVE group was significantly longer than that in Pringle group (27. 0 [20. 0-31. 0] min vs 20. 0 [16. 0-24. 0] min, P<0. 001); the serum ALT levels (P<0. 05) on postoperative day 1, 3, 7 and the serum total bilirubin levels (P = 0. 013) on postoperative day 7 in the THHVE group were significantly lower than those in the Pringle group, and the serum pre-albumin level was significantly higher in the THHVE group than that in the Pringle group on postoperative day 7 (P = 0. 038). The incidence rate of postoperative complication in THHVE group was significantly lower than that in Pringle group (21. 1% vs 37. 5%,P = 0. 032). The overall survival time in THHVE group was significantly longer than that in Pringle group (P = 0. 036). Multivariate analysis by the Cox proportional hazard regressionmodel showed that THHVE was one of the independent factors affecting overall survival, and the death risk of the patients in THHVE group was 47. 6% that of the patients in Pringle group. Conclusion THHVE is a safe and effective method in liver resection for patients with HCC, and the method is associated with less intraoperative bleeding, better postoperative liver function recovery, lower incidence rate of complication and better overall survival compared with Pringle maneuver.

ABSTRACT

Objective To investigate the treatment and prognosis of patients with papillary thyroid microcarcinoma. Methods The clinical and following-up data of 124 patients with papillary thyroid microcarcinoma treated at the Department of Head and Neck Surgery of Cancer Centre, SUN Yat-sen University from Jan 1990 to Dec 1999 were analyzed retrospectively for mortality and survival rate ( KaplanMeier). A multivariate analysis was performed in these patients by Cox proportional hazard model. Results The overall 10-year and 15-year survival rate of all 124 patients with papillary thyroid microcarcinoma were 94.9% and 92.5% respectively. The univariate analysis showed the prognostic factors significantly influencing the survival of patients included age (being worse for those of 35 years and older) at presentation, and the status of distant metastasis ( all P < 0. 05 ) , while gender, incidentally found at surgery, the size of primary tumor, the status of neck lymph node metastasis, recurrence after a primary resection, and the extent of surgical resection did not significantly effect the prognosis. Multivariate analysis showed the age more than 35 years at presentation was an independent risk factor indicating worse prognosis (P = 0. 045 ). Conclusioas The prognosis of patients with papillary thyroid microcarcinoma is satisfactory, but that is poor for those patients 35 years old and up at presentation and with distant metastasis.

ABSTRACT

<p><b>OBJECTIVE</b>The aim of this study was to investigate the susceptibility and prognostic implications of the cyclin D1 gene (CCND1) G870A polymorphism to nasopharyngeal carcinomas (NPC) in Han population in Yunnan China.</p><p><b>METHODS</b>Two hundred and forty one cases with NPC and 271 matched cancer-free controls were genotyped for the CCND1 G870A polymorphism by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and sequencing. The adjusted odds ratios (OR) and 95% confidence intervals (CI) were calculated by using unconditional logistic regression model. Overall survival was assessed using univariate and multivariate analyses.</p><p><b>RESULTS</b>Contrast with homozygous CCND1 G870G, A allele significantly increasing risk of NPC was associated with homozygous A870A (OR = 4.79, 95% CI 2.77 - 8.28, P < 0.001) and heterozygous A870G (OR = 1.72, 95% CI 1.10 - 2.68, P = 0.017). The subjects at least having one CCND1 870A allele had OR of 2.40 (95% CI 1.59 - 3.63, P < 0.001). Furthermore, smoking may increase the risk of developing NPC interacting with CCND1 G870A polymorphism. Kaplan-Meier analysis and Cox regression analysis demonstrated that the five-year survival rate of subjects with AA, AG and GG genotype was 56.2%, 78.5% and 81.4% (AA vs GG, P = 0.003; AA vs AG, P = 0.012; AG vs GG, P = 0.132), but not independent prognostic factor in NPC (P = 0.501).</p><p><b>CONCLUSIONS</b>The CCND1 870A allele is associated with the NPC in Han population in Yunnan China, meanwhile, showed a significant prognosis for those patients.</p>

ABSTRACT

Objective To explore the immune function of the whole body and the tumor site of human nasopharyngeal carcinoma (NPC) patients as well as its correlation with CCR4. Methods The ratios of CD4~+CD25~+T cells and CCR4+cells to lymphocytes were measured by flow cytometry in tissues (25 cases) and peripheral blood (35 cases) from nasopharyngeal carcinoma patients, and then statistical analysis was made. Results The ratios of CD4~+CD25~+T cells and CCR4+ cells in tissue and peripheral blood of NPC were higher than those in the control group (P<0.05). In NPC the ratios of the two cells in tissue were higher than in peripheral blood respectively (P<0.05), but there was no such difference between tissue and peripheral blood in the control group (P>0.05). The ratio of CD4~+CD25~+T cells in tissue at stage Ⅲ+Ⅳ was higher than that at stage Ⅰ+Ⅱ of NPC (P<0.05), but there was no such difference between the two stages in peripheral blood in NPC. There was a positive correlation between CD4~+CD25~+T cells and CCR4+ cells in tissue and peripheral blood of NPC (P<0.05). Conclusion NPC patients have different immunosuppression, and there is even more severe immunosuppression in the tumor site. The ratio of CD4~+CD25~+T cells is correlated with the stage of NPC. Therefore, as NPC progresses to later stages, the percentage of CD4~+CD25~+T cells is increased, which is correlated with poor prognosis. CCR4 plays an important role in reactant of CD4~+CD25~+T cells to tumor sites, and is resistant to immunosurveillance.

ABSTRACT

<p><b>OBJECTIVE</b>To detect the expression of vessel endothelial growth factor C (VEGF-C) and microlymphatic vessel density (MLVD) and probe the relationship between lymphangiogenesis and lymph node micrometastasis in nasopharyngeal carcinoma.</p><p><b>METHODS</b>The expression of VEGF-C, was evaluated in 58 cases with nasopharyngeal carcinoma and 20 samples of nasopharyngitis tissues by immunohistochemical staining SP methods. The MLVD in tumor was counted by immunostaining with the specific antibody lymphatic vessel endothelial hyaluronic acid receptor 1 and analyzed with clinicopathologic parameters.</p><p><b>RESULTS</b>The positive rate of VEGF-C was 84.5% and 15.0% respectively in nasopharyngeal carcinoma and nasopharyngitis tissues, there was significant difference between them (chi2 = 32.309, P <0.01). The microlymphatic vessel density was (28.6 +/- 6.2) pieces per field and (10.5 +/- 3.0) pieces per field respectively in nasopharyngeal carcinoma and nasopharyngitis tissues, there was significant difference between them (t = 12.491, P <0.01). The expression of VEGF-C and the MLVD were significantly higher in nasopharyngeal carcinoma and in lymph node metastasis group than in no-metastasis group. The expression of VEGF-C was positively correlated with MLVD (t =3.512, P <0.01), lymph node metastasis (chi2 =7.715, P <0.01, r =0.712) and clinical stage (chi2 =4.250, P <0.05, r =0.481).</p><p><b>CONCLUSIONS</b>In nasopharyngeal carcinoma, the expression rate of VEGF-C was high. VEGF-C expression was positively correlated with MLVD and lymph node metastasis and tumor staging. It was explained that VEGF-C attached itself to the emergence and infiltration and transfer of NPC. There was close correlation between the expression of VEGF-C and MLVD. It was explained that the expression of VEGF-C in NPC was correlated to neoplasm lymphangiogenesis and that VEGF-C played a vital role in the progression of NPC. VEGF-C was likely to a potential target of anticancer therapy.</p>

<p><b>OBJECTIVE</b>To provide anatomic data for clinical use of reversed submental island flap in reconstruction of buccal and facial defects.</p><p><b>METHODS</b>Twenty cadavers with 40 sides of head and neck which were fixed by formalin and perfused by red emulsion, 6 fresh cadavers which were perfused by coloring agent were dissected, observed and measured.</p><p><b>RESULTS</b>The diameter of the submental artery, the superior lip artery and the inferior lip artery at their origins were respectively (1.42 +/- 0.30) mm (x +/- s, same at below), (1.34 +/- 0.35) mm and (1.34 +/- 0.27) mm respectively. The realistic distance from the origin of inferior and superior lip arteries to the origin of submental artery were (50.13 +/- 13.79) mm and (92.09 8.73) mm, respectively. However, the straight distances from the origin of inferior and superior lip arteries to the origin of submental artery were (35.19 +/- 9.18) mm, (64.99 +/- 5.24) mm, respectively. There were rich anastomoses between both facial arteries, and the facial arteries also anastomosed richly with the ophthalmic artery and the superficial temporal artery. On the marginal mandibular plane, the facial vein ran closely with the artery, the marginal mandibular branch of the facial nerve crossed superficially to the facial blood vessels; superior to this plane, the vein ran 1 cm behind the artery. The buccal branches of the facial nerve crossed superficially to the facial vein and ran into the muscles behind or across the facial artery. The facial vein anastomosed richly with the ophthalmic vein and the maxillary vein.</p><p><b>CONCLUSION</b>There is anatomic feasibility for the reconstruction of buccal and facial defects by using pedicled submental reversed island flap. The reversing point of the flap ought to be at the cheilion plane or inferiorly. Anatomy, regional</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effect of induction chemotherapy on the patients with moderate tongue squamous cell carcinoma and to investigate the factors that influence prognosis of these patients.</p><p><b>METHODS</b>One hundred and twenty two patients with moderate tongue squamous cell carcinoma (stage II-III, T2-3 N0/T1-3N1), treated from Jan. 1990 to Dec. 1999 were retrospectively reviewed. Among them, 69 and 53 patients were received operation alone and operation after induction chemotherapy respectively [cisplatin + 5-fluorouracil + bleomycin-A5 (PBF), 17 cases; bleomycin-A5, 36 cases]. Survival rate was estimated by Kaplan-Meier method. Multivariate analysis by the Cox proportional hazard model.</p><p><b>RESULTS</b>The mean follow-time of all patients were (79.9 +/- 49.8) (x +/- s) months (range: 7 to 177 months), and 45 patients died (including 5 lost to follow up) , 66 of 77 patients alive followed more than 5 years. The overall 3-year and 5-year survival rate were 79.4% and 69. 0% respectively. The overall 3-year and 5-year free-disease survival rate were 71.7% and 66. 3% respectively. The survival rate of 3-year and 5-year was 82.5% and 73.1% respectively for the group of operation alone; 82.4% and 70.1% respectively for the group of operation after induction chemotherapy with PBF, 72.2% and 61.1% respectively for the group of operation after induction chemotherapy with bleomycin-A5; and there were no significant difference between the above three groups (chi2 = 0.42, P = 0.8106). The locoregional recurrence rate were 30.4%, 41.2% and 38.9% for the operation alone group, operation after PBF induction chemotherapy group and operation after bleomycin-A5 induction chemotherapy group respectively. No significant benefit on decreasing locoregional recurrence (chi2 = 1.148, P = 0.563) or distant metastasis rate (chi2 = 2.305, P = 0.316) were found by induction chemotherapy by univariate analysis. Using multivariate analysis, risk factor that independently influence survival was the recurrence.</p><p><b>CONCLUSIONS</b>Risk factors that independently influence survival of moderate tongue squamous cell carcinoma was the locoregional recurrence. No significant benefit on improving survival rate or decreasing locoregional recurrence or metastasis rate were found by induction chemotherapy, there was no difference between the two induction chemotherapy schemes on the survival rate or locoregional recurrence or metastasis rate of these patients.</p>
